A therapy is only as good as its adherence.

External trigeminal nerve stimulation is an emerging non-drug option.

Its strongest evidence is pediatric. A sham-controlled trial in 62 children aged 8–12 (McGough et al., 2019) found a moderate benefit over sham. That trial led to FDA clearance of the NeuroSigma Monarch device for children.

In adults, the picture is genuinely unsettled. A rigorous sham-controlled adult trial found no significant advantage over a placebo device on core ADHD symptoms, and adult research is ongoing. We think the more promising adult question is a different one: whether nightly stimulation helps the sleep that daytime stimulants disrupt.

Whichever way that evidence lands, it will only be answerable if the therapy is actually used and measured. Adherence is what separates a responder from a non-responder — and it's the first thing that falls apart when someone is tired. Vesper closes that loop.

A patient app logs each session, prompts a short daily diary, and administers validated screeners. A clinician view surfaces adherence and symptom trends across the caseload, flags patients below the adherence target, and structures the follow-up conversation.

Vesper is the companion platform, not the device. We don't make the stimulation, and we make no claim that it works.

Nightly session log

Each session captured with start time, duration, intensity, electrode placement and device — plus the streak and adherence figure it feeds. Missed nights are detected server-side; patient-facing reminder delivery is still in build.

Symptom diary + screeners

A short daily diary — attention, sleep, mood, side effects — plus validated screeners (ASRS-5, ASRS v1.1, PSQI) administered in the app and scored server-side against published thresholds.

Clinician dashboard

Caseload view flagging anyone below the CMS-style adherence target, per-patient adherence history and symptom trajectories, and structured, append-only note capture for follow-up visits.

Consent + eConsent

Informed consent workflow that meets enrolment where it happens — in clinic or at home. Signatures are recorded append-only with timestamp, document version and device details; a withdrawal is a new record, never an edit.
